A hospital discharge can arrive with very little warning. One day your loved one is in a hospital bed, and the next you are asked where they will go. Knowing what to ask can turn a stressful moment into a plan.

Start the Conversation Early

Ask the care team about discharge as soon as you can. The sooner you know the likely date, the more time you have.

Questions to Ask the Hospital Team

  • When is the expected discharge date, and what could change it?
  • Will my loved one need help at home, short-term rehab, or a longer stay somewhere?
  • What equipment, medications, and follow-up visits will be needed?
  • Who is the discharge planner or case manager, and how do I reach them?
  • What warning signs mean we should call a doctor or 911?

Get the Home Ready

  • Clear walkways and remove loose rugs
  • Set up a bed and bathroom on the main floor if stairs are hard
  • Stock easy meals and refill prescriptions
  • Arrange rides to follow-up appointments
Write It All Down

Keep one notebook for names, numbers, instructions, and medication lists. When you are tired, it is easy to forget details.

Plan Who Will Help
